Services may be covered in full or in part by your health insurance provider or employee benefit plan. It is the client’s responsibility to check with their insurance company to determine what their financial responsibility is for therapy sessions before the initial session.

Any copay or deductible that is not collected at the time of session will be charged to the client’s credit card on file once the Explanation of Benefits is received by Joseph Montaldi.
